PDHI Launches Back Care Action Plan

Bernardsville, NJ, June 11, 2015. PDHI has added Back Care to its ConXus Steps health action plan library. This behavior-change program helps people with back pain learn coping skills and helps those at risk prevent future episodes. The Back Care action plan can be used as a self-paced learning module or in conjunction with ConXus Direct, an online health-coaching system.

“With pain as one of the top drivers of disability and absenteeism, clients are looking for an effective component to support their population health management programs,” said PDHI President Jennifer Jolley. “This latest addition to our action plan library is an engaging educational experience that’s built on evidence-based treatment guidelines.”

ConXus Steps action plans have received Wellness and Health Promotion (WHP) 7 Certification for self-management tools from the National Committee for Quality Assurance (NCQA). The ConXus Steps library also includes action plans for healthy eating, weight management, physical activity, stress management, depression, diabetes prevention, diabetes management, heart disease prevention, heart disease management, risky drinking, smoking cessation, and financial wellness.

PDHI’s ConXus Platform also includes a WHP-5 certified health assessment (commercial, Medicare, Medicaid), as well as individual and team challenges, a coach documentation system, and extensive tools for managing biometric data and health incentives.
