Project Details
Members log onto the health plan portal and interface to the ConXus applications using a single sign on process. |
| Prescription refill data and laboratory test results are loaded into the PHR from the plan’s pharmacy benefit manager and a national lab testing organization. Members may enhance their PHR by self-entering contact and medical history information to provide a complete record. A health record summary may be printed for sharing with family members and providers. |
| The push messaging module sends online real-time personalized messages to members based on events that take place within the PHR and HRA. For example, members are informed when their lab test results have been automatically loaded into their PHR or when it is time to take a new health risk assessment. The health plan sets the business rules to determine the message triggers. |

| Participants who prefer not to complete the HRA online can request a paper version of the questionnaire. Completed paper questionnaires are sent to PDHI for scanning and loading into the ConXus database for assessment and reporting. |
| An incentive program is in place for which payments are based on employee and spouse (where appropriate) completing the HRA within the enrollment period. |

The standard ConXus Profile HRA questionnaire, assessments, and member report were customized to meet the client’s program goals, while taking advantage of the underlying application framework and tools. |
| Members log onto the health plan portal and interface to the HRA application using a single sign-on process. Patient demographic information, vital signs, and laboratory results are transferred from the electronic medical record system as part of the single sign-on process and are used to pre-populate the HRA. |
| When the member locks and completes the HRA questionnaire, a provider summary report is transferred to the electronic medical record system, where it can be accessed by providers as part of the patient record. |

Project Details
