| ConXus Direct -- Coach Functions |
| Coaches access all coaching tools online from a single, secure portal that provides access to member data, manages workflow, and documents encounters. |
 |
| Member Information |
ConXus Direct provides coaches with immediate online access to all participant data from the member dashboard.
| |
Member information includes program status, demographics, contact preferences, and barrier analysis. |
| |
Data available to coaches collected through other ConXus modules includes:
| - |
ConXus Profile: Review health risk assessment results and, if review mode is configured, enter biometric data prior to locking the health risk assessment questionnaire. |
| - |
ConXus Steps: Enroll participants in action plans, review participant progress, and complete coach-entry steps. |
| - |
ConXus Health: Review both participant-entered and batch loaded data within the personal health record. |
|
|
 |
| Encounter Documentation |
| Coaches can document each encounter with a participant using a Progress Note. Progress notes include contact type, contact method, and encounter information using SOAP notes format. |
| Progress notes are available for review online by participants from the ConXus Program Center. |
 |
| Task Management |
A task-oriented approach is used to manage a coach’s workflow. Task templates may be configured for each client and coaching group to support different coaching models.
| |
Tasks may be assigned automatically by system events, e.g. HRA Locked, Action Plan Started, or assigned manually by a coach. |
| |
The coach’s task management system includes:
| - |
Task lists categorized by status, e.g. Active, Overdue. |
| - |
Task actions for completion of the task. |
|
|
 |
| Scheduling |
The scheduling system manages interactions between the coach and the participant. Interactions may include coaching sessions that form part of a ConXus Steps coached action plan.
| |
Each coach can set his/her availability for coaching sessions. |
|
| |
Tasks can be defined as schedulable by the participant or schedulable by the coach. |
|
 |
| Secure Messaging |
Coaches can communicate online with participants using the secure messaging system.
| |
Participant messages are directed to his/her assigned coach and may include one or more attachments loaded from the participant’s desktop. |
|
| |
Coach originated messages may include one or more attachments from the attachment library or loaded directly from the coach’s desktop. |
| |
Coaches can define text templates for frequently used messages to improve the efficiency and the quality of communications. |
|
| |
Text templates and message attachments can be created and stored in a coach’s personal library or in a shared library for access by all coaches. |
