| ConXus Profile Pediatric is a version of the health risk assessment designed specifically for 6- to 17-year-olds. This assessment looks at a wide range of individual habits and behaviors to identify youngsters who may be at greater risk for health issues. |

The online questionnaire uses conditional branching logic to deliver questions appropriate to each age group.

For the 6- to 12-year olds the questionnaire is relatively short and focused on lifestyle behaviors.

Additional questions are presented to the 13- to 17-year olds to assess risky behaviors such as tobacco and alcohol use. |

When the questionnaire is completed, the responses are locked before the personalized report is displayed, preventing data manipulation. |
