| Health action plans are lifestyle behavior change programs that can be delivered in self-directed mode for personal health improvement, or with ConXus Direct to support coached programs. |

| Action plans can be completed by the participant online. A facilitated option allows the coach to complete the assessment and making a plan module online, and then deliver the personal report and a print-version of learning modules to participants. |
