ConXus Steps -- Health Action Plans
Health action plans are lifestyle behavior change programs that can be delivered in self-directed mode for personal health improvement, or with ConXus Direct to support coached programs.
Action plans can be configured to match client service levels and business rules including the incentive points awarded and the enrollment process. Participants may be enrolled by self-selection, by the health coach, or automatically based on health risk assessment results.

Components
Each action plan comprises:
  Initial Assessment: Measuring risk, lifestyle behaviors, and readiness to make healthier changes

  Making a Plan: Personal plan for improvement, including goal setting and barrier identification
  Personalized Lessons: Interactive learning modules based on goals selected.
  Checking Your Progress: Measuring progress in reaching goals and overcoming barriers
  Final Assessment: Comparing current behaviors with the results of the initial assessment
Delivery Options
Action plans can be completed by the participant online. A facilitated option allows the coach to complete the assessment and making a plan module online, and then deliver the personal report and a print-version of learning modules to participants.
Coach Support
Action plans can be delivered with ConXus Direct to provide integrated coaching.
Program administration is streamlined by task management and appointment scheduling functions.
Coaching sessions are documented through progress notes.
Coaching sessions may take place in person, by telephone, online, or using the ConXus secure messaging system.
Coaches have online access to review participant progress.
Participants have online access to coaching progress notes.
