Members log onto the health plan portal and interface to the ConXus applications using a single sign on process.
Prescription refill data and laboratory test results are loaded into the PHR from the plans pharmacy benefit manager and a national lab testing organization. Members may enhance their PHR by self-entering contact and medical history information to provide a complete record. A health record summary may be printed for sharing with family members and providers.
The push messaging module sends online real-time personalized messages to members based on events that take place within the PHR and HRA. For example, members are informed when their lab test results have been automatically loaded into their PHR or when it is time to take a new health risk assessment. The health plan sets the business rules to determine the message triggers.
Scenario 2
Project:
Online and paper health risk assessment services for 86,000 employees and spouses enrolled in a state employee benefits program.
Employees and their spouses log on to the state employee benefits web site that includes a link to access the online HRA, connecting using single sign-on processes.
Participants who prefer not to complete the HRA online can request a paper version of the questionnaire. Completed paper questionnaires are sent to PDHI for scanning and loading into the ConXus database for assessment and reporting.
An incentive program is in place for which payments are based on employee and spouse (where appropriate) completing the HRA within the enrollment period.
Participants are stratified into three risk groups, based on client algorithms, for enrollment in follow-up disease and wellness programs. Eligibility information is delivered on a periodic basis to organizations providing the follow-up programs.

The health management division provides private label services to several regional third party benefit organizations within the group, as well as providing services directly to employers under their own brand.
Different product lines are supported by deploying ConXus applications independently or in combination, based on configuration options driven by user type. Branding at the user type and employer group level supports multiple private label delivery options.
Access to the ConXus applications uses one of the following methods:
Single sign-on: Participants authenticate and log on at the TPA web site that interfaces to the ConXus servers using the site integration web service API to launch the ConXus application on behalf of the participant.
ConXus Portal: Participants log on through ConXus Portal that authenticates users based on eligibility data supplied by the client on a biweekly schedule.

Online health risk assessment integrated with an Electronic Medical Record system for members of a nonprofit health care system.
The standard ConXus Profile HRA questionnaire, assessments, and member report were customized to meet the clients program goals, while taking advantage of the underlying application framework and tools.
Members log onto the health plan portal and interface to the HRA application using a single sign-on process. Patient demographic information, vital signs, and laboratory results are transferred from the electronic medical record system as part of the single sign-on process and are used to pre-populate the HRA.
When the member locks and completes the HRA questionnaire, a provider summary report is transferred to the electronic medical record system, where it can be accessed by providers as part of the patient record.

Members maintain a PHR in conjunction with an HRA to promote individual health responsibility.
