ConXus® Direct is a health coach portal that provides a cost-effective platform to deliver personalized education and support to participants in health improvement programs. Coaches have access to the following tools:

A queue-based system supports workflow and flexible workgroups. Client-specific alerts notify the coach of high risk participants.

Contact notes allow the coach to document all member interactions.

Online messaging provides a secure platform for member interactions. A client-specific database of standard responses delivers education materials and improves the quality and efficiency of response.
Additional tools are linked to specific ConXus application modules:

ConXus Profile: Coaches can review health risk assessment results and, if review mode is configured, enter biometric data prior to locking the health risk assessment questionnaire.

ConXus Steps: Coaches may assign action plans to participants, review participant progress, and complete coach-entry steps.
•
ConXus Health: Coaches may review both member-entered and batch loaded data within the personal health record.
