ConXus® Health is a fully interactive health record that allows members to compile a personal health history in a secure, central location. Participant self-reported information can be combined with data loaded from third party sources, such as laboratory results and prescription refills. Self-reported data uses SNOMED and NDC codes to provide data portability; loaded data retains the coding used at source.
Member Functions
Components of the personal health record include:
Personal information including demographics, lifestyle, family history, and advance directives.
Comprehensive medication record, including Rx and OTC therapies. Prescription refills can be loaded from PBM data, if available.
Medical history comprising conditions, treatments, allergies, immunizations, screenings, and office visits.
Biometric results can be batch loaded directly from source data, if available.
Provider, pharmacy, hospital, insurance, and emergency contact information.
Printer-friendly health record reports in a variety of formats, including pdf, XML, to allow portability and data sharing.
Administrator Functions
Client administrators are provided with a robust set of online tools for managing projects and reporting data.
A range of online reports deliver utilization and performance data by user type, employer group, and population for each program component.
Data import functions support batch loading of third party data, such as laboratory results and prescription refills, into the health record for access by participants.
Data export functions provide access to all clinical data for detailed analysis of outcomes and integration with other systems.
