ConXus® Profile for adult/commercial populations is a scalable, data-driven health risk assessment. ConXus Profile is compliant with the National Committee for Quality Assurance (NCQA) Member Connections Standards, MEM1.
ConXus Profile Pediatric is a version of the health risk assessment designed specifically for 6- to 12 year-olds. This assessment looks at a wide range of individual habits and behaviors, including nutrition and daily activities, to identify youngsters who may be at greater risk.
Member Functions
A comprehensive assessment is provided from self-reported health status information, personal and family medical history, and biometric data.
The online questionnaire, available in English and Spanish, uses conditional branching logic to provide a personalized and streamlined data collection instrument.
When the questionnaire is completed, the responses are locked before the personalized report is displayed, preventing data manipulation. Optional review mode delays display of the report until biometric data has been entered or loaded.
The questionnaire is also available for paper-and-pencil administration using bubble forms that support standard scanning technologies.
Members completing the questionnaire receive a personalized report, available in English and Spanish, detailing their health risks and lifestyle behaviors, together with recommended actions for health improvement.
A provider summary report is also available.
Scoring algorithms assess modifiable risk factors, disease severity and control, readiness to change, and compliance with recommended preventive screenings. Client-defined risk groupings allow stratification of members into wellness and disease management programs.
Administrator Functions
Client administrators are provided with a robust set of online tools for managing projects and reporting data.
Process management reports combine usage and completion information to track progress and target participants into programs.
Aggregate reports provide an analysis of health risks, profiling the population and identifying highest risk members by employer group.
Batch printing functions support fulfillment of member and provider reports.
Three options are available for entry of laboratory data:
1. Self-report by the participant.
2. Entry by health coaches.
3. Batch load from file.
Tools are available for client administrators to load and process scanned data from paper forms, or PDHI can provide a full outsourced service.
Batch export functions allow clients access to all questionnaire and assessment data for custom reporting and data sharing with partners.
