ConXus Profile v4.4 Health Assessment

Overview

ConXus Profile v4.4 includes:

  • Updated questions for social factors and mood/depression screening for compliance with Healthcare Effectiveness Data and Information Set (HEDIS) 2023 measures.
  • Minor changes to the health equity questions added in v4.3.
  • Updated health risk colors.
  • Additional data export file formats.

Questionnaire Updates

Changes have been made to social factors and depression screening questions to use approved question sets defined in Healthcare Effectiveness Data and Information Set (HEDIS) 2023 measures.

As a result of client feedback on questions introduced in version 4.3 to collect race/ethnicity, gender identity, sexual orientation, and language data required for National Committee for Quality Assurance (NCQA) Health Equity Accreditation standards, “Choose not to respond” and “Other details” response options have been added to these questions.

Assessment Updates

The question updates have resulted in minor changes to the algorithms used to assess risk for:

  • Mood
  • Depression – Current Condition
  • Depression – Future Risk
  • Housing Insecurity
  • Food Insecurity
  • Social Support

Health Risk Colors

A new standard color palette has been developed as part of product UI enhancements. These new colors are used by health risks in the member report (online and printed version) as well as to the health assessment home page widget wellness score graphic.
 

Data Export Files

To better support client requirements for extract of health assessment responses and assessed values, export files are now offered in the following formats:
